Student Immunisation Record

    Ok. Basically, im having a hard time understanding what to do with these forms (C&D). I dont think my GP understands the procedure either.

    I went to my GP earlier today. Got a blood sample taken. Handed him form C.
    On page 11, it clearly states that form C and D should be sent with the blood sample to St.James.

    However, when I rang the medical school I was told that only form C should be enclosed with the blood sample.

    Im confused!:(

    If so you should be fine. I may well be corrected though as I am not a medical student and haven't had to do these blood testy things.

    Yes, I kept form D. To add to the confusion, my old doctor in maynooth has all my records of previous immunisations while my present doctor in castlebar has no proof of immunisation.

    The orientation handbook does a very poor job of explaining the procedures to follow in my opinion.

    And I contacted the medical school by phone so I have no record.

    Any medical students out there who might be able to help me??:(

    Oh there's a few all right ;) It would appear they have changed the format of the immunisation forms this year, could you clarify what's on forms C and D? Here are last year's forms for comparison. At any rate, I would tend to go with what the MSO tell you to do, I would presume they know what they are talking about as plenty of new entrants to health science courses will be contacting them about blood tests at the moment.

    Form C is identical to the student consent form. It allows the result of the blood test to be sent directly to the school of medicine.

    Form D is the student immunisation record, identical to last years one.

    And what exactly is a blood test request form. Must I obtain one of these from St.James's or is that for my GP?

    Yes there is. I'm a Medical student. I did the Vaccinations and Blood Test thing yesterday.
    My GP was no less confused as was yours. Here're what he said to himself when he was filling out the FORM D:"I DON'T KNOW WHAT THE HECK I'M SUPPOSED TO WRITE IN HERE"
    Basically what you do, photocopy the FORM D and let your old GP who has you vaccinations record fill out the form D. The vaccinations that you have to have are Tuberculosis and Hepatitis B. All other vaccinations are not compulsory but are good to have if you want to. If you have never been vaccined against TB and/or Hep. B ask your doctor to do them.
    Your new GP fills out the original FORM D and let him read the FORM A and attach FORM C. All goes in the same envelope including your blood sample and off it goes to the laboratory.
    BTW Even if your Blood test is not proccessed properly you will still start studying on Medicine course regardless with all other Medical students, you just will encouter late registration. So there's nothing to worry about.

    I think me and the GP have sussed things out now. I was in contact with the health service executives national immunisation centre earlier today and they are sending out all records of vaccinations recieved during my primary school days.

    I think all that needs to be attached with the blood samples is form C (The Medical School Office was pretty sure about this too). Then the results are returned to my GP. Then he fills out the Hepetitis B/C results and sends form D to the medical school office attached with original documentation.

    And I dont know if this has happened to anybody else but shortly after giving a blood sample earlier today my GP rang me and told me the Department of Clinical Microbiology at St James's will only carry out tests on blood in a certain type of bottle. So I have to back in again tomorrow which will be another pain in the arm.

    Just surprised really at the lack of information I recieved.:mad:

    Yep, that's what my GP did also.
    I feel sorry for you that you have to repeat the test.
    I'm sure you won't have to worry about it after tomorrow.

    This is the first time my GP had to fill out the form also.
    You live 5 mins walk from O Connell Street and you got a room in trinity halls???:eek:

    And I hadn't a clue about James' saying that they only use a certain type of bottle?? Oh golly....

    Hello Sarah.

    Eh I think they will just send back form D as they dont do anything with it at St.James. Well thats from what I understand.
    And now you can be cool and have two plasters on your arm. Who needs blood anyway!

    My blood was sent to UCD laboratory. It doesn't really matter since international students do their tests in their countries like Canada and their tests are accepted in Trinity.

    By the way here's the Freshers Week schedule specifically for Medical students I made, if anybody needs it (PDF 524kb).

    Jimdw and Kwekubo, I said there weren't too many, not that there are none. When I was in first year there was basically a resident doctor in training on the forum in the guise of Dr. Indy, and there doesn't seem to be the equivlent character. Certainly there is nothing like the saturation of BESS students.

    Anyway mardybumbum I hope that everything gets sorted. Jimdw and Kwekubo seem to have cleared up the confusion.

    Hey Jim, thanks very much for the fresher week schedule. Was meaning to make out my own for a while.:D

    My GP wanted to send my blood to Galway for analysis but I rang the medical school office and they said only to send it so St.James's. :confused:

    Just got out the fee payment form. Thank buddha I dont have to pay the tuition fees! :eek:
