Does anyone know about the fire in Smithfield gate and what happened ? Someone has said that there was only one door to the main stairs from the car park and it was wedged open. Is that right ? Whose fault was it that the door was left open ?

In the first instance- whoever left the door open, and in the second instance- all those who saw the door open and did nothing about it. You can't just blame someone else- anyone who saw it open has at very least a part responsibility for the fact that it was not closed.
