Extreme lower back pain and Dizziness

Hi , first time posting in this section of boards.Dont know if this is the right section but here goes anyways.

Ive been out of work since tuesday with very bad lower back pain.If im sitting down for any length of time say longer than 20mins , when I stand up I get a sudden rush of dizziness and light headed for a minute or two.If I cough or sneeze the pain that I recieve is terrible. Its also hard for me to walk normally without limping. The pain is also affecting my sleep , i keep waking up in agony and covered in sweat.

This isnt the first time that I have had back pain.Around 2 years ago I was suffering extreme back pain and a shooting pain down my right leg.I couldnt move my legs some mornings when I woke up.I went to the doctor and was told that I had sciatica.I had to go to physio for 10 - 12 weeks and was given stretching excercises. Then pain never totally went away and I had to go to dublin to get an MRI. This showed that I had 3 rupured Discs in my lower back and that my pelvis was twisted.I was told that the only option I had was surgery.I was also told that physical labour was out of the question after this for the very distant future.

Due to my work ( Im a mechanic by the Way) I could not afford to give up my day to day job and decided against surgery. Two months after I got the MRI the pain in my back had subsided almost totally.Im 24 and physically fit otherwise.

What do you think I should do , Go back to the doctor or make an appointment with a osteopath or something ?
