Medical Card Application Question

    I'm after recently finding out that I'm eligible for a hse medical card (combination of sucky student stipend and high rent). I've just a few questions about what is needed to apply if people don't mind?

    My name isn't on the lease of the flat I live in. A friend of mine is subletting my room to me. I make a bank transfer every month to his account for rent. Would a bank statement be sufficient as proof of outgoings? Or would I need a letter from my friend? Or would I need a more official letter from the landlord?

    Is a payslip enough evidence of incomings and pps number?

    Now this might sound like a silly question... but does it need to be your GP who fills in part of the form, or can a dentist do it? I'm guessing the likely answer to this, but just in case. :o

    I'd say send in both the bank statements showing rent, and a letter from your friend. One or the other may be sufficient, but by sending in both you will speed the whole process up.

    Payslip is fine.

    It has to be the doctor (he won't charge you for this).
