    Try either Belarc Advisor or look for a copy of Everest Home edition, both will give you an idea of the hardware is in the Medion. As regards modding > Tweaking & Modding would be the place to start.

    If it's 4/5 years old, then it might be hard to get the right RAM for one thing. Possibly DDR1, or older PC133. eBay is a good bet for older components. the 40G disk is probably IDE, you can still get these from Dabs, Komplett, etc. You could open the case and see what graphics card is in there, AGP or PCIe, probably AGP.

    If is everyday browsing, watching films etc... then a new harddrive(if you need more space) and more ram will do the trick.

    If you want to play new games: you are building a new one from scratch and buying ALL the parts new.
    (new case, power supply, new everything)

    Old games are another story.

    It really comes down to what exactly you will use the pc for?
