Desmond speaks to Penny on the phone from the freighter - "Penny? Is that you Penny? Are ya there Penny? I'm on a Boat Penny. I've been living on an island Penny. I love you Penny." - Obviously I've embelished a little but you get the jist

I'll reserve the other 2 nominations until after the Lost Finale.

I loved the whole necklace scene near the end of the "Houses Head" episode of House.

My other two moments are both from the Lost Season Finale (Major spoilers if you haven't seen it)

Lost:

Sawyer sacrificing his chance to get off the island, by jumping from the helicopter, in order to give Kate, Hurley, Jack, Sayid and Frank a better chance at survival

Lost:

Ben reveals to Jack that he can't get back to the island on his own - they all have to go that, including the man in the coffin, who is then revealed to be Locke

Lost- when Ben and hurley were sitting outside Jacob's cabin waiting for Locke. the two never spoke, then Hurley offered Ben some of his chocolate bar. I thought iy was good, very funny, memorable.

Lost- Coffin at the end of season 4 and who was in it.
