hello my phone went bust over the weekend and i got a letter saying i can get a upgrade a few weeks ago.. i want to head to a shop like carphone warehouse or 3 but what do i need to upgrade my phone?? my bill? is that all?

your last 3 bill and proof of Identity- drivers licence or passport...
Not always needed but better to have it than be without and have to make another journey
