Missing Log Book

Does anyone know how I go about getting a new log book. Is this a big deal? I thought mine was at the bottom of my glove box,but on a bit of a clear out found it wasn't

google RF134.
print it out,
bring it to a Garda station and get it signed.
bring the signed form into the tax office, hand over €12(-ish) and they send you out a new one in the post
