if you are looking for perspex/acrylic boxes you could try www.acrylicdesign.ie/.

Also www.alibaba.com - like a b2b ebay... bargains to be had if you are buying in bulk.

as a printer you're obviously working with paper / board / carboard. Why don;t ou just print on thick board paper and make tyour own form a template ?
