Is this setup possible ? ....

    90 cm dish
    Quad LNB on 28 E
    Twin LNB on 19 W
    Twin LNB on 13 W

Is there such a thing as a block for 13/19 with four outputs ?

I don't know of any monoblocks that have 4 outputs for each feed. Monoblocks aren't ideal anyway.

Seperate LNB's would be best. All that should be possible, with a Multi LNB bracket, though you may have to make allowance for the extra weight of the heavier quad and dual LNB's and their relative cables on the arm.

Monoblocks are for a particular size dish in Germany.

    Your original list will work
    Point dish at 19E

I get from 5e to 28e on a 1.m triax dish. The central Lnb is on 16e. I'm open to correction but I think you would need a different type of dish to get from 19w to 28e.
Sorry, I see now you mean 13e,19e, &28e

    Inverto IDLP-23TWMONO and IDLP-23QDMONO.

2 extra questions for those in the know.
Is it the case that you can accomadate more lnbs on a larger regular dish, ie overcome the problem of how close you can mount lnbs together to receive more than 3 birds or is the focal point much the same.
How significant is the exact distance from the dish to lnb (focal length) to good reception and has anyone researched whether varying this could help with close grouping of lnbs.
I found little differece in signal quality moving lnb up to 15cm extra from a 90cm dishs focal point. Is this unusual?

Interesting, Zaphod. Didn't realise such things existed.

Though a report in TeleSatellite reckons that monoblocks are poor compared to seperate LNB's.

Yes. If the arm is longer, the LNBs will be further apart (simple geometry).

Getting the focal point right is important. It you move the LNB further away, it will "see" an annulus which is behind the dish and could be a source of interference.
