Standing Order Question

Hey Folks, Myself and the missus are moving into a new apartment and I have a question. The letting agent gave me a form to fill in which is to do with setting up a standing order. My rather silly question is, do I give this form back to him or do I have to send it onto the bank myself?

I would just ask for the bank account details to pay into and sort it with the bank myself ...

I see no reason to give him a form for this
