Pocket Queens in Early Position

I was running an eye over my Poker Tracker stats today and QQ is the hand that has cost me the most chips over the last month and a half. This is in my Stars STT/MTT database, where I've mostly been playing since the new year. About 70% of the times I've been dealt them its been UTG or UTG+1, so the stats are little skewed.

So I'd really like to get some ideas about how to play this hand better from early pos. I think I decided a few years ago that QQ is one of the few hands you absolutely have to raise with preflop (for a 9/10 seat table), no matter what position you're in. But now I'm not sure.

Is a raise the right move preflop and if so how much (i would tend to raise 3BBs)?

Post flop: If an A or K comes do we continuation bet? If its a flop we like, how do we get action?

I know all this is dependant on stack sizes and the type of players we are up against, but I would like to hear some other opinions on this.

Forgot - if we raise early do we call/push/fold on a reraise?
