Sick Forms??

    Hi all,

    I am seeking help here.
    I am out of work over the next month due to surgery a few days ago.
    My doctor gave me a big blue form with about 4 pages folded around and a small yellow one.
    The blue one is called CB and the yellow one is called BF. He said these are to make sure I am paid. Thing is, I honestly do not know what to do witn them??
    Can someone please help.

    I assume these are the forms for Diability Benefit you are talking about. Depending on your terms of employment, your employer may continue to pay you your wages while out sick, in which case any monies claimed using these forms are generally given back to your employer. If you will not be receiving wages, then you will keep these monies yourself. All of this depends on your PRSI contributions, your employer or tax office will give you these details - the tax office is probably best.

    As regards the forms, AFAIR they are fairly self explanatory to fill out, you just give all your details in each of the sections and the doctor should fill in the specific medical bits for you.

    When thats all done just send them off to your local Social Welfare office and thats it!

    Have a good read over the forms, theres usually lots of info on the "filling in" bit and have a look here on Oasis as well.

    Check with your HR office they'll be able to tell you what to do. As Spocker said it will depend on the terms of your employment among other things.
