Honestly I think you two have a lot to learn about management.
When an error has been made, particularly one that is not especially aggrieving (300k is a drop in the bucket for RTE), it is always better to solve the problem that caused the error rather than to stand there sobbing and pointing fingers.
Lessons will be learned, new policies will be written, and apologies will be made. This is what happens when you run any kind of organisation, public or private. Nobody died like.

For lack of public funding to the tune of €350,000? Yes, people have died. This is public money that went into a smug, sanctimonious, hypocritical liar's pocket when all sorts of public health funding was being cut. Either by long waiting lists or lack of services, people died because this money could have been used for .... just about anything.
