You do according to the premier league:
The champions are given 40 commemorative medals made of silver, which are 2.25in (5.7cm) in diameter.
They can be distributed to the manager, players and officials, as the club see fit, as long as every player who has made at least five Premier League appearances during the title-winning season gets one.
Additional medals may only be presented with the consent of the Premier League Board, and that will only be granted if more than 39 players have made at least five PL appearances during the campaign.

I think it will be really cheap of the club to deny him a premier league medal if the thinking is that it will save a few mill on a bonus.

I think people think that because a player who isn't that good only costs that much that it's nothing to the club. But a couple of million pounds isn't nothing, that's more than what the LFC Foundation raises for a single Legends game every March.
So anyone who has played 5 matches or more gets a medal, and then the club can give out the rest of the 40 they get to who they want. If you have more players than 40 that played 5 matches then more medals will be given, seems to me that is what that says.
At the moment we have 22 players that has played more than 5 matches. We have 6 coaches for the first team and then 16 other staff listed on the staff page of the club. Not sure if all of them is entitled to a medal, I would assume only the coaches would be. That would mean there is around 10 medals that the club can give out to who they want.

So if you have 39 players playing 5 games then that means all + the manager get a medal and you can apply for more to give to other players or coaches
But if its less than that it's expected that 40 medals is enough
But I'd say that's the official rule and you could still apply e.g. if you have 38 players playing more than 5 and a 39th who played 4 it would seem silly not to allow an avenue for extra medals to be awarded to the coaches. But they have to have a line somewhere

Ok for the lads where reading comprehension is your strong suit, the club gets 40 medals, they MUST give one to anyone who has made 5 league appearances, but there is nothing stopping the club giving one to somebody on 4/3/2/1/0 appearances.
