The fact that they are doctors makes sedation slightly more probable. The fact that Kate acknowledges that Madeleine told her she had woken up crying the night before could explain an increase in dosage on the night she disappeared. Nobody has ever suggested that they purposefully injured or harmed their daughter as far as I'm aware. Not even Gerry's sociopathic qualities would suggest that to me. A sociopath would have the arrogance to cover it all up though and convince others to do the same...a refusal to fail.
