diet allowance

hi does anyone know where ive to apply for the diet supplement allowance, is it in the local community care or is it done through the social welfare office.

thanks, according to that ive to go to the cwo in d local health board. does any one have experience with this set up, i.e. do ive to go down every week for it or do they send it out in post.its for my child who needs a special diet.
