So I'm applying for a EHIC, but which place do I send the form to? It says to apply at your local Health Office but is this the same as Heath Center?

Where is the nearest one to the city center?

Also can I just pop in and apply?

    Thanks

You apply on-line using your PPS Number. It will be posted to you.

Tried this earlier before I posted and it said:
The PPS Number,Gender and Date of Birth you have entered do not exist on either the Medical Card or Drugs Payment Scheme database held by the Health Offices.
You will therefore need to apply for the EHIC to your Local Health Office in person or by post.In order to verify your entitlement to Health Services you will need to provide evidence of your residency in Ireland such as a copy of a recent P60, driving licence or pension book.If you have moved to or returned to Ireland to live within the past year, you should provide evidence of your intention to remain for at least a year, such as a rent book or mortgage details, statement from your employer or work permit. If you cannot supply such a document, contact your Local Health Office for more information.
You may choose to print an temporary replacement certificate in the meanwhile

I entered into the HSE Health Centre in Dooradoyle (opposite Hospital) in May for my replacement Card. I just fill out the form, no ID shown, etc. Card two days later in the Post. PPS Number, address and DOB was the only thing needed.
