    Would this be difficult or costly in bike shop. Child really can't use gears as the "twist handle" mechanism seems far too cumbersome. I find the index one far better

    You don't need fancy indexed shifters, something like this thumb-shifter lever would do fine. You'll likely get that style in a bike shop. Fitting is easy enough, you just have to do the cable adjusting once fitted. You'll probably also need new hand grips as the twist shifter will have to go and the grip on that side is shorter.
