Good travel insurance for the USA?

Can someone advise me as to what's the best travel insurance company to go with for a month travelling in the States, ideally one that actually pays up should something happen. As it stands I have no health insurance in Ireland so I'm looking for one that would cover medical expenses if required. Thanks
