    The phrase is a reference to a children's rhyme where a goose flies over a cuckoo's nest and plucks out the chicks. In the story McMurphy plucks the inmates out of the nest of the asylum by making them realise they are capable of independence (in the case of the voluntary acute patients) and by making them believe in their own strength (in the case of the Chief).
    So McMurphy flew over the cuckoo's nest.
