Our consent divorce hearing has had to be put back a month because we didn't have the above form which is needed in Court for the Judge to sign off on - we got our divorce pack from the local Courthouse and no mention was ever made of it.
Its an agreement that you have no legal right to property, pension etc after the divorce is complete - neither of us have a pension or property and the divorce is on complete consent - we have nothing to fight over.
Can anyone either direct me to a proforma on a website or give me a blank copy of something simple they've used as I just can't find anything online and we can't keep taking days off for nothing.
