Passport Office refusing to accept deed poll as proof of name change for 17 year old

    My daughter changed her name by Deed poll with my consent. Passport office are refsuing to accept this and are looking for proof of using new name for 2 years which we dont have.


    Has anyone managed to resolve a similar situation.

    We went through the same with our son. Unfortunately those are the rules. He is in primary school and with COVID we were not travelling so it wasn't such a big deal. We got his school to change his name (alternate spelling) on the school "Aladdin" portal and on all comms and school reports. This was deemed acceptable proof in the end.

    The whole thing is utterly daft, to change the name you have to show proof of a changed name!!, talk about a catch 22, crazy altogether. Best of luck. For the moment if you need to travel I think you'll have to go with the old name on her passport.

    Going down the route of getting a new birth cert (and then applying for the passport) might be the way to go come to think of it as the deed poll may meet the criteria of "over government records" - see below (form letter sent to us)


    Dear Parent(s), Date: 


    In reference to your application to register / amend / add the forename(s) on your child’s 

    entry of birth, please return the following items to this office :



    A fully completed and signed CRA 16 Form



    Original Documentary Evidence in support of the amendment:


    Examples of documentary evidence:

    (proof that the child is using the forename(s) as far back as possible)


    Church Records (Baptismal Certificate/Naming Ceremony Certificate)

    School or Crèche Records

    Public Health Records 

    Other Government Records



    Cheque or Postal Order for € 5.00 

    (made payable to Civil Registration Service)



    The CRA 16 form must be signed by both parents.  


    If one parent is deceased the death certificate must be provided.  


    If the mother’s name only appears on the birth entry then only the mother needs to sign the CRA 16 form. 


    If both parents are deceased a Section 65 form must be completed by the 

    person the birth entry relates to.  They must send this form to: 

    The General Register Office, Government Offices, Convent Road, Roscommon, Co Roscommon, in order to have their forename(s) added or amended.


    Please allow ten to fifteen working days for the amendment in the Register of Births.

    The current plan is to look for the passport to be issued in the name on the birth certificate. However the Passport online system wont let me launch a child consent form in that name. On the advise of a local TD I have emailed the passport office ( on do not reply email address! ) seeking guidance. I am thinking of completing the paper application and sending it in using the postal label for the online application.
