I'm looking to help a relative with some applications including disability allowance. He is separated from his wife and they no longer live together. They have been separated about 10 years but never got a legal separation. Does he still need to fill out the spouse section on the forms?

Don't think so, isn't there a box to tick for separated, don't think it asks if it's legal or not, not an uncommon situation at all. They might come back and look for proof of living apart and maybe a few questions but that's about it I'd say or has been my experience anyway.
