Primary Medical Certificate

Hi my dad has recently just been granted a disabled persons parking permit. He is 80 has prostate cancer and has poor mobility, has bad arthritis in his hips but especially in his knees and tends to shuffle and really not lift his feet at all. He struggles walking far and has to stop and rest. I'm just wondering about applying for a primary medical certificate for him as he struggles getting in and out of my car and doesn't want to go anywhere as it's a struggle (I take him to all doctors and hospital appointments). He has never seen a consultant about his arthritis or his knees so I'm just wondering do you think he would be entitled to it and if a family member is allowed to the assessment as he's not got the best hearing! Thanks
