Upstairs Hot water turn cold when any other pipe is opened

In my house, upstairs Hot water (tap + shower) turn cold when hot water is turned on, on any other pipe in the house. Also when both upstairs + downstairs radiators are turned on only the downstairs radiator would work. My house is 13 years old. Bit confused. What could be wrong?
