Going from Self Employed to jsa to paye

    Hello

I was working self employed for a number of years and registered for income tax. However the business eventually failed so I deregistered self employed and income tax with the revenue dec 31st 2018. I have been on Jobseeker’s Allowance this year but am due to start a Paye job in the next week or two. What tax form would I need for my new employer or revenue to avoid emergency tax as obviously didn’t receive a p45 with me being self employed.

    Thanks
