Applying for medical card bu don't have GP visit card number

    I'm applying for a medical card and they want to know if I've previously had a medical card or a GP visit card. I had a GP visit card several years ago when I was under 18 living with my parents but don't have the card anymore thus I can't put down the number on the medical card application form. Is this a big issue or will the HSE or my GP have this on file?

    Just write “don’t know” it really isn’t a big deal.

    Its not a big deal. Just tick yes and leave the number blank. They will fill in the blanks seen as you've had one before.

    If youre that concerned ring 1890252919 and they will tell you your old number
