Help in completing Medical Card Form

    I'm helping a family member fill out a form for a medical card. It asks for income. His salary fluctuates, sometimes 3 day week, 5 day week, might work for 12 days on the trot, then maybe 5 days again, just earning basic pay. How does he enter his wages on the form? It asks for weekly amount. Any advice appreciated.

    Average the wage and send in a selection of payslips? Is he on social welfare the weeks he is on 3 days? They will have a record of payments made to him from their side.

    Crikey, no he's not on SW on his days off. He is given just a few days notice that his week will be short. To be honest, we never even thought about SW. If that's a possibility then would he be due a lot of SW from the last couple of years? Thanks for replying.

    I'm no expert but I dont think he would get any back payment for the last few years as there would be no proof.

    Go in to the office and speak to them, he will only get something on the weeks he works 2 or 3 days, nothing for 4 or 5 days weeks.

    No he wouldn’t be due any arrears from SW. If he is working short time then he can ask to be considered for JSA for the days he doesn’t work but it’s a complicated arrangement and involves a thorough means test.

    Hey. Just put any figure as they dont really calculate by the net amount per week.

    They will calculate his wages by dividing the insurable weeks by the gross pay YTD figure. So maybe do the calculations first to see if he will be under the allowance for someone his age

    Be warned you'd literally have to dying to get medical card. I know a few who lost it. One of which got it back only a couple of months maybe less before they passed away.

    It takes long when people fail to provide the relevant documents alright.

    Hate these stories all over facebook about the young girl who had cancer but couldnt get a medical card.. Yeah because her father earned 2k a week.. It is a means test after all
