How to claim sick days to social Welfare?

    Hi All,
    Maybe this is a stupid question but it is my first time with a "long term" illness.
    I had an accident and I went to an urgency clinic where I got a few days off. 
    In any, case the nurse told me that probably I would need more days. She was right.
    So, I called to the clinic and asked for more days and I have over 15 days off.
    The point is that the urgency clinic doesn´t have the necessary documentation for the social welfare.
    I got just a document saying that I am not ready for working signed for a doctor.
    Do you know how and where can I get the doc I need to claim those days in the social welfare?

    Go to your doctor and get them to fill in the forms for you and stamp it. You then fill in your details.

    You then put them in your local office or send to Central office which address is on forms.

    I imagine you mean GP. I dont have one.
    I have just three years in Ireland and I didnt need a doctor until now.
    I imagine I could go to one, Would this doctor sign the forms now that I am recovered?

    Yes they should do.

    See is there a practice that would suit you and get in contact with them.

    Usually what happens is you get a form each week then place them in on a final cert as in finished when you are able to return.
