C licence renewal.

Hello everyone, just a quick question about renewing my category C licence, which is up for renewal very soon. Do I need to have a medical report from my GP along with all the other relevant forms. I can not seem to find a definitive answer, any help would be very welcomed, thank you.

Anything in Category C,D,E needs a medical. just be aware that medicals are for 5 years since 2013.

This is form, also includes eyesight test which is completed by your G P.
