Gear stick popping out of gear

This sometimes happens in my car where the stick will just pop back to neutral. Occasionally accompanied by a grinding sound. This also happened to me in a relative's car, so I figure the problem must be me rather than the cars. What could cause this?

Yes, but what would cause that?

Not putting the stick fully into gear. Push the stick fully home
