At the moment i have it cable tie to hold it in place, it doesn't look broken from the outside. Is this a fail? Do they actually check the fuel cap?

Is the cable tie on the outside? Can you see it?

If you get it closed from the inside, they don't check it at all.

9.Check that a fuel tank cap is present and of the correct type. Check that the fuel cap seal is present and is not damaged

    From the NCT Manual...Item 50

Would a local mechanic be able to fix, or need to order parts?

And how do you think they’ll check the fuel cap?

So does the fuel door open and close normally, just the hinge is a cable tie, or is the cable tie holding it closed?

    Also, what car is it?

I had a cheapo universal plastic cap with the yellow fins for years, was never mentioned in the nct

If it is just the door and the cap is fully secure you will be fine.

Had it missing in previous car and no issues.

Edit:providing there is an access to the cap of course. Just remove the flap for the test.

Remove it for the test, as wonski said.

It passed, just need to replace the side head light.
In doing that after the test i notice they remove the air intake to the engine or just incompetent to notice. I don't know whats worst :)

You mean NCT guys removed your air intake?
    Why would they.

    I strongly doubt it.

    Also I don't think it's a reason for fail.
