Switching back to disability allowance

    Hi all,

    I have recently finished a QQI (fetac) course that was provided by ETB. My disabilty allowance was supposed to be switched back when I finished the course but unfortuntely wasn't. I got in contact with the Disabilty allowance section in Longford and was told I need a DA104 form. I have found one online but it is dated 2014. Will that form do or is there an updated one? If so, where can I get it?
