Applying for full licence and documents required.

    Hi, So i passed my test earlier and im trying to figure out what medical documents i need to bring with me to the NDLS center. Is it just the eyesight form, or is a completed medical form also required ?. And is the eyesight form even required since i completed one less than a year ago when applying for the provisional licence ? 55 euro for the eyesight test:rolleyes:

    What it says on the website is this:
    • Fully completed application form for a driving licence D401
    • Certificate of competency (must be dated within the previous two years)
    • Current or most recent learner permit
    • If you do not have your most recent learner permit in your possession, you must complete the lost or stolen permit declaration on the application form, have it witnessed by a member of An Garda Síochána and submit it with alternative photographic I.D.
    • Proof of your personal public services number (PPSN)
    • Evidence of address dated within the last six months is required if your current address is not recorded on your permit and it differs from that provided to the Department of Employment Affairs and Social Protection (DEASP) if and when you applied for a PSC
    • Evidence of residency entitlement
    • Fully completed NDLS medical report form, dated within one month, if required in your case
    • Evidence of certificate of professional competence (CPC), if required
    • Evidence of initial basic training (IBT), if required
    • Application fee of €55.00

    However, I went in for mine about a month ago and just had my learner permit and that was fine. Could've been I just got the sound guy working there, and he was really sound.

    Thanks. And yep, its not needed for the full licence just the provisional, im just back from applying.
