I've lost my log book and need to replace it.

I have the form and it asks me to tick the correct box for what I want. I need the log book (the book you sign when you sell the car and stick it in the post)

    The car is a 2005

Which dam box do i tick?

Can you send this form off or do you have go to the motor tax office?
    Is the fee still 12e?

You don't need to go to the motortax office. You do however need to pistachio the form to your local motortax office and they then post you a receipt. You then receive a log book a couple of days later from the Shannon Head Office

Fee is still 12 + the cost of a postal money order + the cost of a stamp. Give or take it's more like 14 Euro

No stamp required for government offices.
