VAT and medical equipment

Looking for some advice on VAT and the purchase of medical equipment.
I suffer from sleep apnoea, and need to use a CPAP machine to stop me from not breathing during the night.
I was getting quotes from various suppliers of machines, and they were all quoting me prices, ex VAT @ 23%. As a PAYE worker, I know I can claim back medical expenses at 20% of cost. However, I am not sure if this is applicable to multiyear goods/equipment. Additionally, one of the vendors mentioned that there is a way to reclaim the entire VAT on the machine (about €230), as well as then claim the 20% tax refund on medical expenses.
Could any one enlighten me on if this is possible, and if it is, how I would go about it?
Note: I am a PAYE employee on the marginal tax rate, with about €400 in other (uncovered) medical expenses per year.
