How to fill in Form 11 for paying back tax from receiving employer shares

I'm not sure if it is allowed or even practical for me to ask a question like this here. This is all very unfamiliar to me! I've been on to the tax office and they weren't overly helpful.

Basically, reading IT72 "Tax treatment of shares acquired by employees and directors under Unapproved Share Option Schemes", it turns out I own Revenue some money as I got some SAR's shares from work and although I paid CGT (far too much, it seems), I did not pay any income tax.

Even though I am a PAYE worker, I have a hobby that means I can potentially earn some extra money. This means I have to do a Form 11 tax return every year. I was hoping to clear up this outstanding tax matter this November while doing my Form 11 return.

I rang the tax office to explain what happened and that I'd calculated how much I owed. I asked what should I do next. I offered to call in, but they said there was no point as all they'd do is ask me to fill out a Form 11 if I called in.

So my question is: How and where do I fill in this information on the Form 11? I do my return online at ROS.

Again, apologies if this is not the appropriate place to ask this.

    Much appreciated.
