D201 question eye exam

    Sorry to ask but just in relation to the form D201 for a first time renewal of the learner permit question 18:
    Do you need to wear glasses
    or lenses for driving? Yes No
    (If yes Code 01 will be added to your licence)
    If in the past you answered ‘Yes’ to this question
    and are now answering ‘No’ you must provide a
    current eyesight report with your application.

    I previously answered yes and gave my eye exam results. It's a little vague to whether I need to represent a current exam. If anyone knows the answer (I've tried google but no luck) please let me know :).

    When did you show and is it shown on your licence now that you required glasses.

    The only way you would need to give an eyesight report (or a medical report with the eyesight question answered) on renewal is if you had laser surgery (or some other circumstance where your eyesight had improved to the point) where you no longer needed glasses. As long as you're declaring you still need glasses, you don't need to provide a cert.
