I presume this is change of name by Deed Form

You can email them for the Form. They don't complete it for you.

You have to complete it and have it sworn before a solicitor then have it stamped in the Stamping Office in the Fourt Courts and then hand it in at the Desk where they check it.

If you just turn up you are wasting your time.

Thanks for the reply, hope you don't mind me asking you another question,
on the Affidavit page it asked to insert description/occupation of witness, is it a physical description of the witness or as in my case my sister is going witness for me does she just put this down as description ie I SUCH AND SUCH being the sister of xxxxxxxxxxx
hopefully this is the last question i want to make sure all is right, thanks

It means their Job or you can put a generic title such as gentleman

So it would be Susan X, Nurse of Address etc.
