Question about making a new JSA claim

    Hey, i'm going into the SW office tomorrow to sign on for JSA.
    The first time i go in do they just give me a application form?
    What do i need to bring with me? Is my social card, passport and esb bill as proof of address enough?
    What else count's a as a valid proof of address?
    Thing is i was on the social welfare before up until near the end of last year.
    It's a long story which i'm not going to get on to here but it resulted in my JSA being canceled.
    I'll be going into the office i used to go to, so would they still have my details on the system or would i have to do everything from scratch?
    Really my main question is, do i have to be there to make a new claim or can i get someone to pick up an application form for me.
    And once completed and have all the necessary documents that are required can go in and make a new claim.
    It's just a nuisance having to go in 3-4 times for the same thing, and going in and being told they can't do anything till i bring in X Y and Z.

    Just wondering is that the most recent one, so i can print it out and fill it out ahead of time and get everything sorted in one go?
