    You know you can do this online? no need for a physical form. Just need to send a signed copy once they accept it.

    Hi, I'm not setting one up really but needed to know what forms are required. I was pretty sure it was just this one but just wanted clarification.

    No worries. The form is the correct one as was already said.

    In reality, you would now use core to register rather than a form.
