Just looking for some information, about to gather my reciepts to send off a med 1 form. Have yet to send off for 2014's prescriptions because i only learned about this form recently.

Do i need to send off two separate forms for 2014 and 2015 or is the one okay for both years?

If it's any help, ive spent €1400 in the last 7 months of 2014 on prescription medicine, and genuinely lost count of how much i spent in 2015! But my chemist can provide that info apparently.

    Thanks guys!

You are using an online forum. Don't bother with a paper med 1 form. Use your online paye anytime account to claim tax relief on your medical expenses.

Each year is a separate claim. You need to be accurate with your claim. You need to retain your receipts for 6 years and send them in if requested. Revenue don't need them up front.
