Can someone tell me if someone who is on the disability has a family member as a carer are they also untitled to a HSE carer too?

I'm not quite sure what you mean by 'on the disability' - do you mean receiving disability allowance?

As far as I know, there is no 'general' entitlement to a HSE carer. Some people with disabilities require very minor assistance, and some people require multiple carers around the clock - so it is really case-by-case.
