Employer paid medical insurance relief

So I sent off forms claiming tax back on medical insurance that is paid for in full by my employer. Got a call earlier to clear up an issue with a date on the forms and was informed - as a side note - that I most likely owe them money. Is this possible? Could I have filled in the forms wrong? If anyone can help clear this…

Is this possible? Could I have filled in the forms wrong? If anyone can help clear this up, please PM me. Every time I request a balancing statement from Revenue it seems I owe them money.
