Passport questions regarding birth cert.

    Hi, I need a passport by the 8th of June so I'm going to use passport express. I have everything stamped and signed and ready to go, but I was just wondering about my birth cert. It doesn't have my last name on it, just first and middle. I used this to apply for grants etc before and it has worked fine. I have lost my previous passport so I cannot send that away with the form, but I have a passport from when I was about 7 years old (12 years ago) which is expired. Would I be able to use my birth cert or passport or what do I do ?

    It doesn't have my last name on it, just first and middle.
    Does it have your parent's surnames. Do you use one of those surnames?

    It has my Mother's name, but not my Dad's. But on my old passports and most things I have had my father's surname.
