    I have a pair of Shimano R063 shoes (cheap entry level) that I bought a couple of years ago and had the cleats fitted as part of a bike fit in an LBS.

    The shoes fit fine but once I go over about 4 hours I get very sore under the ball of my foot, a hot spot almost like a very tender bruised sensation. No problems at all for shorter spins or on the turbo. No other leg or knee pain either.

    Is this likely to be bad cleat placement or is it simply a case of cheap shoes not really meant for longer distances and I need to upgrade to something with stiffer soles.

    As much as I love spending money on new cycling gear, there's other things I could be spending it on.

    Try moving the cleat backwards slightly. Most 'how to set up cleats/pedals' articles state to put the cleat square under the ball of the foot but that doesn't suit everyone. I have my cleats as far back as the go on the shoe and that works for me.
