Flying in/out of Pisa and looking at heading to Florence, Siena & Pisa itself. Is there anywhere that I should be adding to/removing from that list? Can anyone recommend any good vineyards/farm stays in any of those?

San Gimignano is a beautiful medieval walled city-worth a half day trip. Also Lucca is a beautiful walled university town-hire a bike at the entrance and view it that way. Siena is beautiful and has a magical atmosphere.

There is nothing in Pisa apart from the Leaning Tower and the buildings beside it so you could do that in a half day. (The leaning tower itself is visible from the plane on the approach into the airport.)

San Gimignano is a must. Fattoria degli Usignoli in San Donanto in Fronzano (near Reggello east of Florence) is a great agriturisomo although it has many of the features of a 4 star hotel.
