    I get the feeling from the report that it was empty, due to a previous problem with likely that very door.

    Very similar accident happened in Miami, in 1989 to a TAMPA Colombia DC-8-63F in July 1989. I belive like the above incident, the cargo door was incorrectly latched, and subsequently opened as the aircraft pressurized during the climb. Aircraft returned safely to KMIA.

    Is that a plug door design? Like could it have happened higher up nearer cruise where the consequences would be a lot more severe?

    Nope unless the door opens inward it wont be a plug door most cargo doors are latched.
