Form 12 - Dividend Witholding Tax - Quick question

    So I'm filling in my form 12 for last year.

    I got dividends which I have declared. They were subject to witholding tax.

    Can anyone please tell me where on the form I enter the DWT amount.... I can't find it!


    I did it the last couple of years, but of course didn't save a copy of them!

    It's a set rate so if they're Irish dividends it either goes into the box "from which DWT was deducted" or the box "from which DWT was not deducted".
